I hereby irrevocably grant to The Company (and its successors, assigns, licensees, employees, agents and contractors) the right in perpetuity throughout the universe, and in all now known and hereafter existing media, to use my physical likeness (“The Photo”) in connection with the production, exhibition, exploitation, merchandising and promotion of The Mommies – A Musical Blog™ and other Company presented or licensed entertainment endeavors. I agree that the foregoing grant (“Authorization”) includes the right to use my name and physical likeness in any form, including, without limitation, a photograph, picture, poster, artistic rendering, silhouette or other reproduction in print or digital photograph displayed in any recorded or reproduced form in any media (fixed or electronic). Such Authorization also includes related marketing and promotion displays, such as on site at women’s health related seminars, workshops, expos, conventions and other public or private events; and, in or on cards or other merchandise on behalf of The Company’s business.
I represent that I am over the age of 18 and that the consent of no other person, firm, corporation or labor organization is required to enable you to use the Photo as described herein and that such use will not violate the rights of any third parties. I hereby release The Company (and their assigns, advertisers, and affiliated parties) from any liability of injury and damages resulting from my participation and grant of rights.
I acknowledge that nothing herein requires you to use the Photo. The rights granted herein include the right to use the Photo and the right to edit, retouch and/or juxtapose the Photo with other text and graphics.
I hereby certify and represent that I have read the foregoing and fully understand the meaning and effect thereof, affirm sufficient consideration (mutual promises of publicity, benefit, performance, and other) and, intending to be legally bound, I authorize The Company to use my Photo.
